Menopause is a naturally occurring biologic process in females, generally occurring in from age 45 to 55. Menopause occurs after the last ovulation marking the end of the fertile period of a female’s life. It is defined as the permanent end of menses and fertility occurring 12 months after a patients last menstruation. During menopause the body regulates itself to acquire a new hormonal balance, spanning for the rest of the lifespan of the female.

When a female goes through menopause the ovaries stop producing eggs and fewer hormones (estrogen and progesterone); menstrual periods generally become either more closely or more widely spaced, but may even come to a sudden stop. This reduction in potent and active sex hormones, and their downstream biological effects, cause a plethora of changes in physiological processes. An irregularity in menstrual cycles can occur for one to three years. Even though a natural process and not a medical illness, it still can have dramatic effects on a women’s mental and physical health. These effects vary dramatically from one patient to another and are primarily caused by the changes in estrogen and progesterone.

Menopause symptoms

The effect of the underproduction of hormones, and other biological molecules, by the ovaries manifest in several unpleasant symptoms. These symptoms can be divided into two categories physical and psychological. Physical symptoms include irregular menstrual periods, hot flashes, night sweats, heavy sweating, fatigue, muscle aches, heart palpitations, headaches, dizziness, sore breasts, sensitive skin, dry hair, hair loss, prolapse, vaginal drying, painful sexual intercourse, vaginal discharge, loss of libido, urine tract infections, urinary incontinence, and an increase in osteoporosis. Psychological symptoms include mood swings, fatigue, apathy, anxiety, and depression.

Menopause treatments

A common treatment used for the symptoms of menopause is hormone replacement therapy. Hormone replacement therapy mimics the human body’s natural production of those hormones in order to reduce the symptoms associated with dramatic swings in hormone concentrations during menopause. Since these treatments are patient specific and vary from patient to patient many different formulations of hormone therapy can be made by compounding pharmacies to meet these individual needs. Hormones of all strengths and concentrations can be put into oral capsules, topical creams or gels, vaginal creams, suppositories, injections, and sublingual lozenges. There are also other prescription alternatives to hormonal therapy that may work for some women. The treatment of these symptoms can be accomplished by replacing the hormones that the underperforming ovaries fail to produce. This therapy is referred to as hormone replacement therapy (HRT). The body naturally stops producing these hormones because its genetics dictate the time for reproduction is over. Even though the body is programmed this way, replacing hormones in the appropriate amounts can maintain a comfortable balance that is beneficial to the female.

Vasomotor symptoms are usually described as night sweats, hot flashes, and flushes. Hormone replacement therapy can reduce these vasomotor symptoms in postmenopausal and perimenopausal women by 65% and 90%, respectively.

There are many different drugs prescribed to treat the symptoms of menopause but not many drugs to treat the underlying causes of menopause.

The lack of hormonal signals, and in balance initiates the biological processes that produce the symptoms mentioned above. Therapies to treat and replace the hormonal signals are readily available from qualified physicians.

Therapy includes mono or combination therapy of the hormones progesterone, estradiol and in some cases low dose testosterone. Even though estrogens are referred to as the female hormones, every female also is genetically set to produce testosterone in small amounts. Serum levels of testosterone also drop as estrogen concentrations decrease. Hormone replacement therapy replaces the hormones that the ovaries fail to produce naturally. Generally, if the risk reward ratio of hormone replacement therapy is in the favor of the patient’s health, due to the disruption from menopausal symptoms, hormone replacement therapy might be considered by your physician.

Hormone replacement therapy is not for all patients, many patients with pre-existing conditions should consider alternatives if concerned with adverse effects. The benefits of a comfortable and healthy lifestyle should be weighed against the adverse side effects of hormone replacement therapy by your physician.

In recent news, hormone replacement therapy has become the subject of debate among medical communities. A large shift in prescribing patterns recently occurred in the general physician prescribing population. When hormone replacement therapy was first prescribed to treat menopausal symptoms, it was generally regarded as safe for all, almost equivalent to consumption of over-the-counter daily vitamins and supplements. Generally, physicians prescribed hormone replacement therapy because of the dramatic effect on the mood and well-being. However, dramatic psychological and physical beneficial effects came at a large cost to some. Few women prescribed hormone replacement experienced moderate to severe side effects. These effects did not occur in the large majority of women, but were at a large enough percentage to warrant other drugs with less side effects as treatment for the symptoms of menopause. Many women continue to take hormone replacement therapy for treatment of menopausal symptoms.

Predisposition to disease plays a large part in determining if a patient is an appropriate candidate for hormone replacement therapy. Additionally, lifestyle and dietary changes are considered the first line of treatment for healthily undergoing the changes of menopause.

Secondarily, medications with outstanding safety profiles are considered for the alleviation of menopausal symptoms before hormonal therapy is considered. However, contrary to popular prescribing patterns, some physicians prefer to utilize hormone replacement therapy as a primary treatment for menopausal symptoms, these physicians generally approach cautiously. Physicians using hormone replacement therapy as a primary treatment thoroughly evaluate a patient’s pre-disposition to adverse side effects, determine precise hormone dosages, and closely monitor the patient during initiation and continuation of therapy. These measures increase the safety of hormone replacement therapy, a knowledgeable physician can monitor a patient and determine physiological changes occurring from the hormones administered. If physiological changes are determined to be dangerous to the patient’s health the prescribing physician can cease hormone therapy. The serum markers indicative of adverse effects should be monitored by a prescribing physician. Generally, these biomarkers include cardiovascular, liver, and general metabolic proteins and small molecules associated with the adverse side effects of estrogen, progesterone and testosterone administration. Negative effects on inflammatory markers biomarkers include, E-selectin, cell adhesion molecules, monocyte chemoattractant protein-1, and tumor necrosis factor-alpha, inconsistent effects on interleukin-6, and stimulatory effects on vasoprotective cytokine, such as the transforming growth factor-alpha. However, women who receive 2-3 years of HRT after menopause do not have increased mortality, in contrast cardiovascular benefits are apparent when compared to those who had not been treated with HRT.


Taking care of daily responsibilities, managing both a career and family, and sticking to a healthy diet can sometimes make it difficult to put a smile on your face, much less to get in the mood for sex. Loss of libido is an unfortunate condition that affects millions of women on a daily basis. Women from young to old can experience low sex drive at various stages of life: younger woman may experience it after the birth of a child in their 20’s and 30’s; perimenopausal woman usually during their late 30’s and 40’s; and both menopausal and postmenopausal woman throughout the rest of their lives can all experience low libido.

Loss of libido is a complex phenomenon with psychological, relational, physical, and hormonal dimensions as unique as the women who experience them. The term libido has long been used to describe a person’s sexual drive and their desire for sex. Loss of libido, medically termed “hypoactive sexual desire disorder,” is a reduction or lack of interest and desire in sexual activity. Loss of libido is chiefly characterized by a lack of interest or desire for sexual activity and many women with loss of libido find that they are less in touch with their sexuality–sexual feelings come less frequently and energy for sex drastically dwindles or disappears fro ma woman’s life.

Causes Of Low Libido

Many factors can play a role in a woman’s drop in sex drive, which may be primarily caused by changes that are hormonal, physical, psychological, or relational. More specifically, some of the contributors to low female libido include: decreased estrogen levels; vaginal dryness; medication side effects; chronic health conditions; loss of a spouse or partner; lack of emotional intimacy; conflict; stress; and mood concerns.

A decrease in sex drive can develop due to medical conditions or as the result of psychological or emotional issues. Inhibited sexual desire is a type of sexual dysfunction that affects both men and women. A reduction in sexual desire is most often associated with diminishing levels of testosterone in aging men, and menopausal transitions (reduced estrogen levels) in women who may also be sensitive to reduced testosterone levels. Chronic illnesses and chronic pain can also lead to a decrease in sex drive, likely through a combination of physical and psychological stressors. Additionally, certain medications such as some antidepressants can also reduce libido.

Symptoms Of Low Libido

The symptoms of low libido often mimic those experienced during and menopause and afterwards, and can range from non-existent, to mild, to severe. Common menopausal symptoms include:

  • Hot flashes
  • Sleep disturbances (insomnia)
  • Emotional changes, such as mood swings or irritability
  • A change in sexual interest or response
  • Problems with concentration and memory that may be linked to sleep loss and/or fluctuating hormones
  • Headaches
  • Rapid, or irregular heartbeat (tachycardia)

After you stop having menstrual periods, you may get other post-menopausal symptoms include, the most common of which include:

  • Drying and thinning of the skin, caused by lower collagen production
  • Vaginal and urinary tract changes
  • Vaginal dryness, irritation, and itching
  • An increased risk of vaginal and urinary tract infections (UTIs)
  • Pain during sexual activity

Treatment Of Low Libido

Some medications work to increase vaginal lubrication and relax vaginal muscles, where as others contain testosterone to increase sex drive4 or estrogen to improve sensation, lubrication, and sexual interest. A simple balancing of different hormones can also be the solution. Topical libido enhancers are an appealing alternative to oral medications. Inconsistent dosing and possible side effects can be eliminated with topical creams that are applied locally to increase sensitivity and blood flow.

  • Pentoxifylline 12.5mg – causes changes in your blood that help improve blood flow, which helps your blood carry oxygen to your tissues and organs. It is used to improve blood flow and reduce certain symptoms of a condition called intermittent claudication. It is also purported to have similar libido enhancing effects on women.
  • Sildenafil Citrate 2.5mg – relaxes muscles and increases blood flow to particular areas of the body. Sildenafil under the trade name Viagra is used to treat erectile dysfunction (impotence) in men, and is purported to have similar vasodialation effects on clitoral artery blood flow.
  • Testosterone 0.25mg – is a sex hormone that is produced by males in large amounts, and is also produced in small amounts within women. Testosterone is used in men and boys to treat conditions caused by a lack of this hormone, such as delayed puberty, impotence, or other hormonal imbalances. It has also been used in hormone replacement therapy to improve sexual function in premenopausal and postmenopausal women.

The inability to conceive a child can be stressful and frustrating, and although infertility affects almost 6.7 million couples in the United States (a staggering 10-11% of reproductive aged couples in the United States), it is highly treatable in many cases.

The highly specialized field of female fertility involves a wide range of medical, environmental, and lifestyle causes which also includes many very specific risk factors. However, now that the genetic causes of female infertility are more commonly diagnosed, and several female infertility treatments are readily available, couples having difficulty conceiving, or carrying to term, can often have success with the use of fertility medications.

How is Infertility Defined?

Infertility is defined as a couples’ inability to conceive a child even though they’ve had frequent, unprotected sexual intercourse for a year or longer. According to the Mayo Clinic, infertility results from male infertility factors about one-third of the time, female infertility factors about one-third of the time, and unknown or a combination of male and female factors about one-third of the time.

Are There Other Symptoms of Female Infertility?

As would be expected the main symptom of infertility (male or female) is the inability to conceive. However, if you are a woman age 30 or older, who has had unprotected intercourse for six months to year without getting pregnant, you should have an infertility evaluation. Other symptoms may include:

  • an absent menstrual cycle indicating lack of ovulation
  • an irregular menstrual cycle that’s too long (35 days or more) or too short (less than 21 days)
  • an abnormal menstrual cycle that’s heavier or lighter than usual
  • history of irregular or painful periods, or pelvic inflammatory disease
  • repeated miscarriages
  • prior cancer treatment
  • endometriosis

Most physicians agree that when you seek medical attention should be age-dependent, and that if you’re

  • in your early 30s or younger, try to get pregnant for at least a year
  • between 35 and 40, six months of trying
  • if you’re older than 40, a couple of months

Causes of Female Infertility

As you can imagine a number of female factors can disrupt any step of this process, and female infertility may be caused by one or more disruptive factors. More specifically, infertility causes are traditionally grouped into categories among which are: ovulation disorders; damage to fallopian tubes (tubal infertility); endometriosis; and uterine or cervical causes.

Ovulation disorders represent ovulating that is infrequent or non-existent, and can be caused by flaws in the regulation of reproductive hormones, the hypothalamus, the pituitary gland, or by problems within the ovary itself. Ovulation disorders account for infertility in approximately 25% of infertile couples, and includes:

  • Polycystic ovary syndrome (PCOS) – complex changes occur in the hypothalamus, pituitary gland and ovaries, resulting in a hormone imbalance, which affects ovulation; most common cause of female infertility.
  • Hypothalamic dysfunction – disruption in the release of the two pituitary gland produced hormones responsible for stimulating ovulation each month, i.e., follicle-stimulating hormone (FSH) and luteinizing hormone (LH); main sign of this problem is irregular or absent periods.
  • Premature ovarian insufficiency – a disorder usually caused by an autoimmune response wherein your body mistakenly attacks ovarian tissues or by the premature loss of eggs from your ovaries due to genetic problems or environmental complications like chemotherapy.
  • Excessive prolactin – rarely the pituitary gland (or medication contraindications) can induce the excessive production of prolactin (hyperprolactinemia), which reduces estrogen production and may cause infertility.
  • Damage to fallopian tubes (tubal infertility) or blockage, prevents sperm from getting to the uterus and egg for fertilization, causes include:
  • Pelvic inflammatory disease – an infection of the uterus and fallopian tubes due to chlamydia, gonorrhea, or other sexually transmitted infections.
  • Previous surgery in the abdomen or pelvis, including surgery for ectopic pregnancy – a fertilized egg becomes implanted and starts to develop within a fallopian tube instead of the uterus.
  • Pelvic tuberculosis – a major cause of tubal infertility worldwide, although uncommon in the United States.

Endometriosis – occurs when extra tissue growth occurs within the uterus requiring surgical removal which can cause scarring that may obstruct the tube and can:

  • Prevent egg fertilization
  • Affect the lining of the uterus, disrupting implantation of the fertilized egg
  • Affect fertility indirectly by damaging the sperm or egg

Uterine or cervical causes – can impact fertility by interfering with egg implantation or increasing the likelihood of a miscarriage through:

  • Benign polyps or tumors (fibroids or myomas) are common in the uterus

The Clinical Rational For Treating Female Fertility

The cause, your age, how long you’ve been infertile, and your personal preferences all largely determine how your infertility is treated. The complexities of infertility constitute significant time, psychological, physical, and financial commitments. Although some women need just one or two therapies to restore fertility, it’s possible that several different types of treatment may be needed before you’re able to conceive.

Treatments attempt to restore fertility in three primary ways:

  1. Although rarely used now due to the overwhelming success rates of other fertility treatments, several surgical procedures can correct problems or otherwise improve female fertility, including laparoscopic or hysteroscopic surgery, tubal ligation reversal surgery (microscopic), and basic tubal surgeries.
  2. The most commonly used method is reproductive assistance intrauterine insemination (IUI), and assisted reproductive technology also known as in vitro fertilization (IVF).
  3. The primary treatment for women who are infertile due to ovulation disorders is fertility restoration, or stimulating ovulation with fertility drugs which regulate or induce ovulation. In general, they work like the natural hormones, follicle-stimulating hormone (FSH) and luteinizing hormone (LH), to trigger ovulation. They are also used in women who ovulate to try to stimulate a better egg or an extra egg or eggs.
